"U.S. Hospital Errors Continue to Rise"

MONDAY, April 2 (HealthDay News) -- Patient safety incidents in U.S. hospitals increased by three percent overall from 2003 to 2005, and the error gap between the nation's best- and worst-performing hospitals remained wide, a report released Monday found.

America's top rated centers had 40 percent lower rates of medical errors than the poorest-performing hospitals, the study showed.

The fourth annual HealthGrades Patient Safety in American Hospitals Study, put out by HealthGrades, an independent health care ratings company, examined over 40 million Medicare hospitalization records at almost 5,000 hospitals from 2003 to 2005.

The study found that:

There were 1.16 million patient safety incidents among Medicare patients during the three years of the study.

That works out to an incidence rate of 2.86 percent.

During those three years, there were 247,662 potentially preventable deaths in U.S. hospitals.

Medicare patients involved in one or more safety incidents had a 25 percent chance of dying.

The excess cost to Medicare associated with patient safety incidents was $8.6 billion from 2003 to 2005.

Ten of 16 types of patient-safety incidents increased over the three years of the study, by an average of almost 12 percent.

The greatest increases were in post-operative sepsis (about 34.3 percent); post-operative respiratory failure (18.7 percent); and selected infections due to medical care (about 12.2 percent).

Incidents with the highest occurrence rates were decubitus ulcer; failure to rescue; and post-operative respiratory failure.

If all hospitals had performed at the same level as the top-rated hospitals, about 206,286 patient safety incidents and 34,393 Medicare patient deaths could have been avoided, resulting in $1.74 billion in savings.

"The cost of medical errors at American hospitals in both mortality and dollar terms continues to be significant, and the 'chasm in quality' between the nation's top and bottom hospitals, which HealthGrades has documented in this and other studies, remains," the study's primary author, Dr. Samantha Collier, HealthGrades' chief medical officer, said in a prepared statement.

"But the nation's best-performing hospitals are providing benchmarks for the hospital industry, exercising a vigilance that resulted in far fewer in-hospital incidents among the Medicare patients studied," Collier said.

Some assert that there is no end to desire, so we should undercut our ambition.

But this doesn't address the need for satisfaction.

We need to have satisfaction in what we do in order to have a good sense of well-being.

If we undercut our ambition, then we will never make any achievements nor satisfy our yearnings.

This only leaves us with frustration, uncertainty, and timidity.

Therefore, to follow Tao (path of life's ever-continuing evolution), we must identify our inner longings AND DISPATCH THEM, WITH A HUNTER'S ACCURACY.

"Out here", outside of CYBERSPACE ...

I am a "real" person who does have contact with other younger, and sometimes, not-so-young people about this or that ...

"Life issues", as it were ....

And in those conversations, I do "dispense" many of these same thoughts or aphorisms, perhaps, that appear in here, so as to assist these people in "un-confusing" themselves ...

Since no one "external" to them can really do that job for them ......

And just recently, I had a conversation with a younger person who I have known since he was quite young ...

And what I said to that person was that, in my own observations, this person had spent a long time developing himself, to "be the best you can be" ...

And now, perhaps, it was time to simply "be" .....

You can pursue perfection so long and hard that you never really have time to just be a simple human-being ...

And this is in part because "being the best you can be" is undefined ....

How do you know that you are ever "there"?

And so ...

To which I replied that life is eminently fair in that it is completely indifferent to us all ....

If there is "unfairness" in life, it is a "concept" that is self-created ....

Quite literally, it is because they cannot breathe ....

They are "stuck" on what I will call the "eternal inhale", so that in essence, and in actual fact, they are "holding their breath" all the time ....

And it is surprising how "unaware" people are about this "condition", even as it "afflicts" them to their detriment ....

Which thought I am going to continue on with in here ....

Which is one of the methods of getting the actual "breathing process" back on track ....

Wherever we are, the constant flow of Tao (path or course of life's continuing evolution) is ever present.

We see the cycle of opposites, such as the juxtaposition of sunlight and snow.

We notice the ongoing rhythms of life, waterfowl carrying on their lives even as spring is slow to warm and leafless trees stand in anticipation of warmer weather.

ALL THINGS CHANGE, ALL THINGS MOVE CONSTANTLY.

The world is like the ongoing turning of a magnificent wheel.

They can give us the guidance and support to face our fears.

Although they can neither live our lives nor solve our problems outright, they can provide an invaluable presence just when we most need it.

Within ourselves, our daily devotions are the way to encourage ourselves to persevere.

Sound is the same as sight, taste is the same as smell, touch is the same as thought, and all that we are is identical with the spiritual energy that resides within us.

In this high concentration, there is complete union, and we feel the joy of total integration with all our facets.

These are all indications that there is a separate way of looking within.

In meditation, it is important to discover and utilize this mode of introspection.

We must go beyond thought, go beyond visualization, go beyond imagination and actually open a part of the mind that most people leave dormant.

This inner eye has a location, buried deep in the brain.

When it is opened, it is our way of receiving more subtle experiences than we receive in our physical states.

Perhaps looking and seeing are misleading terms, after all.

We don't necessarily "see" images through this inner eye.

We gain direct awareness that is beyond the image.

Pineal gland

From Wikipedia, the free encyclopedia

The pineal gland (also called the pineal body or epiphysis) is a small endocrine gland in the brain.

It is located near the center of the brain, between the two hemispheres, tucked in a groove where the two rounded thalamic bodies join

Location

The pineal gland is a reddish-gray body about the size of a pea (8 mm in humans), located just rostro-dorsal to the superior colliculus and behind and beneath the stria medullaris, between the laterally positioned thalamic bodies.

It is part of the epithalamus.

The pineal gland is a midline structure, and is often seen in plain skull X-rays, as it is often calcified.

Structure and composition

The pineal body consists in humans of a lobular parenchyma of pinealocytes surrounded by connective tissue spaces.

The glands' surface is covered by a pial capsule.

The pineal gland consists mainly of pinealocytes, but four other cell types have been identified.

The pineal gland is the only brain structure that does not come in pairs.

Cell type Description

The pinealocytes consist of a cell body with 4-6 processes emerging.

They produce and secrete melatonin.

The pinealocytes can be stained by special silver impregnation methods.

Interstitial cells are located between the pinealocytes.

Many capillaries are present in the gland, and perivascular phagocytes are located close to these blood vessels.

The perivascular phagocytes are antigen presenting cells.

In higher vertebrates neurons are located in the pineal gland.


However, these are not present in rodents.

In some species, neuronal-like peptidergic cells are present.

These cells might have a paracrine regulatory function.

The pineal gland receives a sympathetic innervation from the superior cervical ganglion.

However, a parasympathetic innervation from the sphenopalatine and otic ganglia is also present.

Further, some nerve fibers penetrate into the pineal gland via the pineal stalk (central innervation).

Finally, neurons in the trigeminal ganglion innervates the gland with nerve fibers containing the neuropeptide, PACAP.

Human follicles contain a variable quantity of gritty material, called corpora arenacea (or "acervuli", or "brain sand").

Chemical analysis shows that they are composed of calcium phosphate, calcium carbonate, magnesium phosphate, and ammonium phosphate.

Recently, calcite deposits have been described as well.


Function

The pineal gland was originally believed to be a "vestigial remnant" of a larger organ (much as the appendix was thought to be a vestigial digestive organ).

It was only after the 1960s that scientists discovered that the pineal gland is responsible for the production of melatonin, which is regulated in a circadian rhythm.

Melatonin is a derivative of the amino acid tryptophan, which also has other functions in the Central Nervous System.

The production of melatonin by the pineal gland is stimulated by darkness and inhibited by light.

The retina detects the light, and directly signals and entrains the suprachiasmatic nucleus (SCN).

Fibers project from the SCN to the paraventricular nuclei (PVN), which relay the circadian signals to the spinal cord and out via the sympathetic system to superior cervical ganglia (SCG), and from there into the pineal gland.

The pineal gland is large in children, but shrinks at puberty.

It appears to play a major role in sexual development, hibernation in animals, metabolism, and seasonal breeding.


The abundant melatonin levels in children is believed to inhibit sexual development, and pineal tumors have been linked with precocious puberty.

When puberty arrives, melatonin production is reduced.

Calcification of the pineal gland is typical in adults.

Pineal cytostructure seems to have evolutionary similarities to the retinal cells of chordates.

Modern birds and reptiles have been found to express the phototransducing pigment melanopsin in the pineal gland.

Avian pineal glands are believed to act like the suprachiasmatic nucleus in mammals.

Reports in rodents suggest that the pineal gland may influence the actions of drugs of abuse such as cocaine and antidepressants such as fluoxetine (Prozac); and contribute to regulation of neuronal vulnerability.

Mythologies, cultures and philosophies

The secretory activity of the pineal gland has only relatively recently become understood.

To philosophers, its location deep in the brain seemed to indicate its importance.

This combination led to its being a "mystery" gland with myth, superstition and metaphysical theories surrounding its perceived function.

René Descartes, who dedicated much time to the study of the pineal gland, called it the "seat of the soul", believing it is unique in the anatomy of the human brain in being a structure not duplicated on the right and left sides.

The pineal gland is occasionally associated with the sixth chakra (also called Ajna or the third eye chakra in yoga) or sometimes the Seventh (Crown) chakra.

It is believed by some to be a dormant organ that can be awakened to enable telepathic communication.


The relevance of the pineal gland to Discordianism, an idiosyncratic religion with roots in California psychedelic culture whose doctrines display great fondness for paradox, is great if not well understood.

Writers such as Alice Bailey, considered an early proponent of the new age movement, use the pineal-eye as a key element in their spiritual world-view...(see Alice Bailey: "A Treatise on White Magic")

The notion of a 'pineal-eye' is also crucial to the philosophy of the seminal French writer Georges Bataille, which is analysed at length by literary scholar Denis Hollier in 'Against Architecture' (1990, trans. Betsy Wing).

In this work Hollier discusses how Batailles' uses the concept of a 'pineal-eye' as a reference to a blind-spot in Western rationality.

"Obesity rising in Europe, especially in children"

BUDAPEST (Reuters) - The number of overweight people in Europe is rising and there is an especially worrying trend of increasing childhood obesity and in the number of people who are grossly obese, according to recent studies.

Europe is facing major health and social burdens and the rise in obesity is reaching "epidemic" proportions, the 15th European Congress on Obesity in Budapest was told on Sunday.

Estimates show there are around 1.1 billion overweight people in the world, of whom 312 million are obese, and that in Europe 10-20 percent of men are obese and almost half the population is overweight.


Some 30 percent of children in Britain are obese or overweight, and percentages are rising in southern Europe, while in new European Union states in eastern Europe, rates of obesity are surging at a time when health spending is being curtailed.

"More than 80 percent of children who are already obese will stay obese as adults," said Martin Fried of Prague's Charles University, who authored a major study on the effect of surgery on obese patients.

Fried estimates that there are around 11 million Europeans who are grossly obese, with a body mass index of 40 or more.

Body mass index (BMI) is a measure of body fat based on height and weight in which normal weight is in a BMI range of 18.5-24.9.

Obese people are far more likely to suffer early deaths, have health problems like type-2 diabetes, and have a lower quality of life, as well as being unable to participate in work, Fried told journalists on the sidelines of the conference.

Fried said surgery was increasingly a workable solution for obese adults and that far from competing for health resources, it was better than for example continuing to treat type-2 diabetes with drugs, as surgery offered a once-and-for-all solution for 80 percent of cases after six months to a year.


In Britain, which has one of the highest levels of childhood obesity in Europe, a study showed that community-based programs had a major impact on overweight and obese children.

Results of a 9-week trial program conducted by University College London (UCL) and Great Ormond Street Hospital for Children involving 107 families showed significant improvements in BMI, waistlines, lifestyle and self-esteem.

These results were sustained over 12 months, the study said.

"Obesity costs the nation 7 billion pounds a year."

"This popular community-based program has the potential to underpin effective national strategies for obesity treatment and preservation," said Professor Alan Lucas of UCL's child health department.

As long as one's deeds are in accord with the time and one leaves no sloppy traces, then the action is correct.

Violinist Katherine Fong, a native of Amarillo, Texas, began playing at age 4. She has been a member of the Metropolitan Opera Orchestra since 2000. She has served as Principal 2nd Violin under the baton of Plácido Domingo and has played with many of the greatest singers in the world . Comfortable in many musical genres, Katherine is eager to lead a multifaceted existence as an orchestral musician, chamber musician and rockstar violinist. Katherine's 2006-07 season in addition to the Met opera season included an appearance as soloist with the East Texas Symphony Orchestra in a premičre of Robert Sirota's "Epiphanies" for solo string quartet and orchestra; featured violin soloist on The Today Show with Andrea Bocelli; backup violinist to Jill Scott on Late Night with David Letterman; playing electric violin, and a 5-string Zeta, as well as singing backup vocals with various rock bands in NYC clubs and on tour in Ireland; emulating Jean-Luc Ponty with The Mahavishnu Project at BB King's; and a string quartet recital at The Bruno Walter Auditorium at the New York Public Library for the Performing Arts at Lincoln Center.

Katherine has toured Japan, Germany, and Switzerland with the Met Orchestra and has performed with the Met Chamber Ensemble and Maestro James Levine at Carnegie and Zankel Hall. As a founding member of the Acacia String Quartet (1998-2000), Katherine was a finalist in the 1999 Young Concert Artists Competition, winning the Baerenreiter Prize. The quartet also participated in the 1999 Isaac Stern Workshop, where its first performance on the stage of Carnegie Hall was praised by the New York Times as being full of "youthful enthusiasm and sheer technical proficiency". As a soloist, Katherine has given recitals at Harvard University and Columbia University's Miller Theater. She has participated in the Aspen, Taos, Salzburg and Luzern summer festivals.

Katherine received a Bachelor of Music from the New England Conservatory, where she studied with Eric Rosenblith, and continued her studies with Cho-Liang Lin and Joel Smirnoff at the Juilliard School, where she received her Master of Music degree. Katherine currently lives in Soho in New York City and when not playing the violin enjoys acting classes, Italian language classes, yoga and an obsession with HBO On-Demand.

Wei-Yang Andy Lin, Viola

Wei-Yang Andy Lin, violist, born in Kao-Hsiung, Taiwan, is currently a 1st year Master's Degree student at the Juilliard School studying with Hsin-Yun Huang. He came to the U.S. in 1997 to attend Idyllwild Arts Academy and studied with Donald McInnes.

He has won numerous competitions including the Tainan County and City competitions, Kao-Hsiung City Gun-Du-Bae Competition, Taiwanese National Viola Competition, the Idyllwild Concerto Competition, and currently won 2nd price in the Juilliard Viola Concerto Competition. Mr. Lin has been a member of the LDX String Quartet, appearing on NPR's "From the Top" radio broadcast; Kafka Piano Quintet, appearing at New York's Taiwan Center and New Chinese Television; and the Rimbaud String Quartet, with whom he was awarded a fellowship to attend the Aspen Music Festival. Mr. Lin has served as a principle violist in the Idyllwild Arts Chamber Orchestra, Ureuk Symphony, and the Bethany Church Chamber Orchestra. He has also had the privilege to perform for the President of the United States, Bill Clinton and the President of Taiwan, Ten-Hui Lee.

He has performed in Carnegie Hall, Avery Fisher Hall, Alice Tully Hall, Juilliard Paul Hall and Morse Hall, Juilliard Theater, Merkin Hall, Kennedy Center in Washington, Whitaker Center in Pennsylvania, CUNY Graduate Center, Greenfield Hall in Manhattan, Mannes Concert Hall, Los Angeles Music Center, New York Taiwan Center, Queens College Concert Hall, and Tainan Cultural Center in Taiwan.

He has been a part of several Music Festivals which includes Bowdoin Summer Festival, Aspen Summer Music Festival, Music Academy of the West in Santa Barbara, and The Perlman Chamber Music Workshop. Throughout his musical study, he has studied chamber music with Earl Carlyss, Warren Jones, Cathleen Winkler, Timothy Eddy, Jerome Lowenthal, Joel Sachs, Arnold Steinhardt, Andre Emelianoff, and Seymour Lipkin, and has studied solo with Donald McInnes, Roger Myers, Victoria Chang, John Graham, Michael Tree, Toby Appel, and currently with Hsin-Yun Huang at the Juilliard School.

Year 2006, he was accepted to both Mannes and Juilliard for master's degree program with full scholarship, and has decided to stay in Juilliard for his master's degree and study with Hsin-Yun Huang. He has been invited to perform with Itzhak Perlman and play Mozart Viola Quintet as a first violist.
